CHIP/Children's Medicaid Application Drive
Tarrant County families with uninsured children may be able to get health coverage through the Children's Health Insurance Program (CHIP) or Children's Medicaid.
Benefits include choice of doctors, regular check-ups, prescriptions, dental services, shots, eye exams and glasses, and more.

To apply, bring the following:
- Your child's Social Security number.
- Child's Alien Registration Card (I-551) or a copy of the front and back of the card if your child is not a U.S. citizen.
- Proof of each family member's income, such as most recent paycheck stub, a letter from an employer stating how much is earned, or papers that show how much is being received in child support.
- Proof of how much your family pays for childcare, disabled adult care or child support.
