2010 events in the Dallas-Fort Worth area

To apply, bring the following:

  • Your child's Social Security number.
  • Child's Alien Registration Card (I-551) or a copy of the front and back of the card if your child is not a U.S. citizen.
  • Proof of each family member's income, such as most recent paycheck stub, a letter from an employer stating how much is earned, or papers that show how much is being received in child support.
  • Proof of how much your family pays for childcare, disabled adult care or child support.
